A lost or stolen passport must be reported to the State Department. Once reported it is cancelled, even if you find it later.
Steps
- Report it with form DS-64 (online, by mail, or in person together with your new application).
- If it was stolen, a police report can help, but it is not required to apply.
- Apply in person with form DS-11 plus DS-64 at an acceptance facility, with proof of citizenship, photo ID and a photo.
- Pay the same fees as a first passport ($165 for an adult book). See fees.
Traveling soon?
With international travel within 14 days, book Los Angeles Passport Agency, San Diego Passport Agency and San Francisco Passport Agency. Otherwise add expedited service ($60): expedited passports.
Where to apply in California
Use any of the 583 offices: Los Angeles, San Diego, San Francisco, San Jose, Sacramento, Fresno, Roseville and Long Beach, or all cities. For ID while you wait for a passport, see REAL ID vs passport.
Questions
Can I use my passport if I find it after reporting it lost?
No. Once reported lost or stolen, it is cancelled and cannot be used for travel.
Do I need to go in person?
Yes. A replacement for a lost or stolen passport is a new application with form DS-11, made in person.
